Improving care trajectories of all older persons living with dementia: avoidable hospital use, primary care and propensity scores.
Persons with dementia have twice as many hospitalizations as older persons without dementia. Hospitalizations in persons with dementia are associated with increased risk of adverse outcomes, including impaired quality of life, delirium, falls, cognitive and functional decline, long-term care admission, and death. A share of these hospitalizations might be avoidable. In this presentation, I will share three studies I conducted in Qc to describe service use and explore promising intervention to reduce avoidable hospital use in persons with dementia. I will then present my current projects on the diversity of persons with dementia and the impact of the pandemic on their use and care.
